- Valid, unexpired government-issued photo ID — such as a driver’s license, state ID, passport, or other acceptable identification.
- Service authorization or registration documents — if your service was requested by an employer, school, court, attorney, agency, or other organization.
- Donor pass or electronic registration information — for drug and alcohol testing, if provided.
- Fingerprinting instructions or fingerprint cards — including an FD-258 card, authorization form, agency code, or reference number, if provided.
- Court or legal documents — if your testing or service is related to a court, probation, custody, or other legal matter.
- Collection kit or laboratory order — for phlebotomy or specialty collection services, when applicable.
- DNA testing documentation — including any testing instructions or legal paperwork, when applicable.
- Documents requiring notarization — bring the complete document and any required witnesses, if applicable. Do not sign beforehand if your signature must be witnessed by the notary.
- Payment method — for any fees due at the time of service.
