At BC Secure Solutions (BCSS), we are required by law under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), 45 CFR Parts 160 & 164, to protect your health and personal information.

1. Information We Collect (45 CFR §164.501)

Demographic Information: name, date of birth, address, phone number, email, employer, and emergency contact information.

Service Information: details related to specimen collection (DOT or non-DOT), notary services, or workplace programs.

Health Information: limited medical information necessary for specimen collections, testing, and compliance with federal/state/DOT regulations.

2. How We May Use and Disclose Your Information

For Treatment: performing collections, confirming identity, and sharing results with Medical Review Officers, employers, or agencies (45 CFR §164.506).

For Payment: billing you, your employer, or a contracting agency for services provided (45 CFR §164.502(a)(1)(ii)).

For Operations: quality assessments, staff training, and compliance checks (45 CFR §164.501).

As Required by Law: disclosures for audits, investigations, or public safety (45 CFR §164.512).

We will never sell or disclose PHI for marketing purposes without written authorization (45 CFR §164.508).

3. Your Rights Regarding Your Information

Access: You may request a copy of your records (45 CFR §164.524).

Amendments: You may request corrections to inaccurate information (45 CFR §164.526).

Restrictions: You may request restrictions on use or disclosure (45 CFR §164.522).

Complaints: You may file a complaint if you believe your rights have been violated (45 CFR §160.306).

4. Website & Online Demographics (45 CFR §164.502)

Your demographics are securely transmitted and stored when submitted online.

Information is used only to process your request, confirm your identity, and provide services.

Access is limited to authorized BCSS staff who need it to perform their duties.

We do not use your website-submitted data for marketing without your consent.

5. Safeguards & Security

Administrative safeguards: policies, procedures, and staff training (45 CFR §164.308).

Physical safeguards: restricted access and secure storage (45 CFR §164.310).

Technical safeguards: encryption, secure access controls, and secure transmissions (45 CFR §164.312).

6. Complaints (45 CFR §160.306)
