For Treatment: performing collections, confirming identity, and sharing results with Medical Review Officers, employers, or agencies (45 CFR §164.506).
For Payment: billing you, your employer, or a contracting agency for services provided (45 CFR §164.502(a)(1)(ii)).
For Operations: quality assessments, staff training, and compliance checks (45 CFR §164.501).
As Required by Law: disclosures for audits, investigations, or public safety (45 CFR §164.512).
We will never sell or disclose PHI for marketing purposes without written authorization (45 CFR §164.508).

Your demographics are securely transmitted and stored when submitted online.
Information is used only to process your request, confirm your identity, and provide services.
Access is limited to authorized BCSS staff who need it to perform their duties.
We do not use your website-submitted data for marketing without your consent.
